it looks like wound dehiscence (separation), which is a very normal and manageable complication. I had similar separation at my posterior fourchette (near where yours seems to be) around the 1-2 week post-op mark; I shared some progress photos after my own bottom surgery last year. my surgical team told me that that happens with a huge plurality of patients that undergo vaginoplasty, simply because of the forces at work on that portion of the perineum.
if you're losing bright red blood from that opening or there's a nasty smell or discharge, that could be an emergency situation. but absent those signs, it may very well be an expected complication. in any case you should absolutely talk to your surgeon.

There’s something very wrong if that’s normal if that’s the way he normally does surgery. You could tell him you posted it here on reddit and everyone says it looks bad. This sub is the by far the largest trans surgery resource in the world and Dr Hess is going to damage his reputation if he doesn’t do something about it.

As mentioned, the second hole separation can occur and isn’t not necessarily a major concern, albeit a complication.
It is hard to tell from these photos and your description alone, and may well be in the realm of normal and manageable complications. Yet, given your description, and the amount of bright red blood in the picture, you may need to go to the ER.
How much is it bleeding?
That looks like a sizable drip in the first picture, and may include general drainage, but if it’s regularly spitting bright red blood, go to the ER!
the blood is coming out of a little wound I have on my labia. Thought the hole (fistula) was bleeding. they‘re gonna fix it when I get my revision

Biggest concern is infection or bleeding with the fistula. So you are likely ok if it isn’t bleeding significantly from the fistula itself.
Sounds like you are in good hands. But please go to the ER if you ever do start bleeding significantly from the fistula, or if it starts swelling significantly, feels extra warm, tender, etc. You will likely be fine though.

My best guess would be the hole is to a rectovaginal space; between the vaginal canal, and the rectum. In any case, I would be very concerned about infection if its bleeding this much still, this needs seeing to from your surgeon.

Extremely concerning that your surgeon/care team is not responsive to you post-op. I’ve heard this a lot.

Honey, if it's fistula please seek a treatment immediately. Untreated fistula, of any kind is dangerous.

it‘s apparently just a fistula. they will fit it when I get my revision in 8 weeks. the blood is also from another little wound I have on the right side of my labia.

When is your follow up surgery scheduled? I’d be a little concerned about continued bleeding out of the fistula. If it’s a small amount of blood, that’s not a big deal, but a lot of blood loss from a wound that isn’t closing can be dangerous.
FYI: one of the main increases in force at that area (posterior forchette) is while pooping (big increase in pressure through the abdomen and pelvis). If you have a squatty potty and exhale as you poop - make sure you don’t hold your breath when you initiate pooping - it will help decrease that pressure. Opioids (common pain medication for major surgery) can make constipation worse, which leads to more straining while you poop. If you are in fact having this issue (which is very common after surgery), ask if they can adjust your meds to help as well. They can give you stool softeners and other meds to help alleviate the side effects of opioids.
