My vagina now produces a fetid smell, oh god, the worst thing I have ever smelt, do you recommend me to change my douche? I am only using water. It smells like rotten fish, I was noticing it a bit while masturbating, and then I smelt the dilator and I almost vomited

That sounds like a vaginosis. You should use antibacterial douche agents to get this cleaned up. And once this has been neutralized you should aim to get good lactobazilli in there and on lowering the ph down there.

what should I douche with? and then I just go to the pharmacy and ask for lactobacilli for the vagina or what do I say?

Ask a local gyn to help you. Its a common problem like with yeast infections. Iodine sokution added to the douche water might help without a prescription drug. There are pills to help with getting a good vaginal flora as well as vaginal creams. You can ask your pharmacist to get you informed about the products available to you.

Try a douche that is water-vinegar mix to see if that helps. If it isn't enough, try water mixed with some already-diluted hydrogen peroxide.

Vaginal smells usually are caused by buildups of bacteria (this can happen with cis women's vaginas too, not just trans women's) and plain water isn't always enough to clean things out, but those two mixes should help kill and remove a significant amount of the foul smelling bacteria.

My obgyn prescribed me a metronidazole pill twice a day for about a week to clear up my case of bv. Beforehand I had always douched once a week with a vinegar/water mix, but she told me I should really just be using a light soap and warm water mixture.

There's a lot of things it could be. Vaginosis, yeast, etc. it's hard to know exactly what it is unless you toss a sample under a microscope. This happens to cis women all the time, and you go in, get seen, and the doc tells you what it is, and what can fix it.

Sounds like an infection, BV as others have stated. Please seek advice from a GP or GYN, infections left untreated can make you very sick.

That's normal. We don't produce our own vaginal flora like actual women do. We need to douche with vinegar according to my surgeon.

That's normal.

It's actually not, and shouldn't be expected to be a given. I'm closing in on 2 years post-op, and I've never had a BV or yeast infection.

We don't produce our own vaginal flora like actual women do.

Fucking yikes. I shouldn't even have to point out how incorrect (and downright transphobic) this statement is. Trans women are actual women, period.

Bacterial vaginosis. This is something you need to see your gynecologist for.
